Joshua offers a wide variety of family medical services, serving patients of all ages, including children & infants.


Joshua is a board-certified Family Nurse Practitioner by the American Academy of Nurse Practitioners.


Joshua was born and raised in Tennessee. He earned a Bachelor of Science in Nursing from Cumberland University, a Master of Science in Health Care Informatics from Lipscomb University, and a Master of Science in Nursing from Herzing University. He began his career in healthcare in 2009 and has experience with bedside nursing, nurse management, healthcare technology, and medical research.


Joshua and his wife reside in middle Tennessee and have one child. In his spare time, he enjoys exercising, sports, being outdoors, and spending quality time with his family, including their two golden retrievers.


Joshua joined the Summit Primary Care family in July 2024 and is currently accepting new patients!

Bridgett was born and raised in Northwest Alabama. She moved to the Hermitage area in 2003 to start her career as a registered nurse. After serving this community as an ER nurse for 18 years, Bridgett made the decision to expand her knowledge and start a new path as a Nurse Practitioner.

 
Bridgett loves to use natural and alternative medicine when appropriate, combining it with traditional medicine to develop care plans focused on the health and wellness of her patients. Bridgett focuses on family medicine and provides care to all ages, including children. 

Dr. Pak was born and raised in Huntsville, AL. He earned his undergraduate degree from Emory University in Atlanta, GA. He completed his medical degree from the University of South Alabama in Mobile. He then returned home for his residency in family medicine through the University of Alabama in Birmingham. He is a member of the American Academy of Family Physicians.

Dr. Pak joined Summit Primary Care in August 2005. His Practice covers the full spectrum of family medicine, including pediatric and preventive medicine. He is currently accepting new patients of all ages.

Dr. Pak currently lives in the Hermitage area.

Dr. Hamby was born and raised in the Cincinnati, Ohio area. She earned her undergraduate degree from Emory University. She completed her medical degree at Ohio State University College of Medicine. Dr. Hamby then completed her residency in family medicine in York, PA. She is a board certified family physician with the American Board of Family Practice.

Dr. Hamby places a high value on developing a good relationship with each patient and strongly believes that caring for the entire family is extremely rewarding. She provides a full spectrum of primary care to all age groups and particularly enjoys working with children. Dr. Hamby is currently accepting new patients of all ages.  Her Medicare and TennCare panels are full, so if you are a new patient with either of these insurance plans, then you may select another one of our providers for your PCP.

Michael Warren is a TN licensed and Board Certified Physician Assistant (NCCPA). He has 15 years of Family Medicine and Urgent Care experience providing a full spectrum of services to all age groups. He is currently accepting new patients of all ages. Michael places a high value on developing a partnership of healthcare with each patient. He also finds reward in caring for the entire family.

Michael was born in Northwest Indiana. He received his undergraduate degree from MTSU in Murfreesboro, TN and his Physician Assistant degree from Barry University in St. Petersburg, FL. He practiced in Tampa, FL as a family physician assistant until joining SPC in January 2015.

Michael currently resides in East Nashville. His favorite hobbies are oil painting and graphic design.

Dr. Gaston, Jr. started his Family Practice with Donelson Medical Clinic and has served this community for over 40 years. He was a founding physician of Summit Primary Care when we opened in January 2000. He sees patients of all ages from newborns to seniors.

Dr. Gaston, Jr. is a graduate of the University of Tennessee Medical School. He interned at the City of Memphis Hospital, where he also served as Chief Resident. He began practicing in the Donelson-Hermitage area in 1977. 

Dr. Gaston, Jr. was born in Lebanon and raised in the Donelson area. He lived in Old Hickory for many years and currently resides in Nashville with his wife, Jane. They have two married sons, Hugh and Jon, and 3 grandchildren to date.

Dr. Milligan offers a full spectrum of family medicine services with no age restrictions , including pediatric and preventive medicine. He also specializes in providing joint injections and vasectomies. He is currently accepting new patients of all ages, and walk-ins are welcomed. 

Se habla Espanol. Dr. Milligan is fluent in Spanish and is looking forward to serving that community in our service area.

Dr. Milligan was born and raised in the Seattle, WA area. He completed his medical degree at Loma Linda University. He completed his residency in Family Medicine through the University of Nebraska.

Experience: Family Physician at Nellis AFB in NV: 1997 until 2001. Private practice at Moses Lake Clinic in Moses Lake, WA: 2001 - 2013. He is Board certified and a member of the American Academy of Family Physicians.

Dr. Milligan currently resides in the Mt. Juliet area with his wife and 2 children.

Dr. Major's practice focuses on Family Medicine, with special focus on lipid/obesity disorders and diabetes. He sees patients of all ages from newborn through seniors. He is actively accepting and recruiting new patients to his practice, and he welcomes all new patient referrals from current SPC patients.

Dr. Major did his Medical Internship and Medical Residency at the Mayo Clinic Family Medicine Residency Program in Des Moines, Iowa. He practiced Family Medicine at Mayo Clinic in Des Moines from 1998 until joining Summit Primary Care in July 2000.

Dr. Major is board certified by the American Academy of Family Physicians.

Dr. Major was raised in Lebanon, TN and currently resides with his wife, Angie and their son, Jackson, in Mt. Juliet on Old Hickory Lake.

Amanda Fallos is a board certified Physician Assistant who joined Summit Primary Care in January of 2012.

She earned her Bachelor of Science degree from the University of Illinois at Chicago.  She completed her Master of Science degree as a Physician Assistant from Trevecca Nazarene University in Nashville, Tennessee.  Amanda is also a member of both the Tennessee and American Academies of Physician Assistants.

Amanda's experience as a provider includes working at an Internal Medicine practice specializing in Gastroenterology and Hepatology in Nashville, Tennessee.  

Amanda speaks Spanish and is happy to serve our non-English speaking Hispanic community.
